why not try calling into a residential home for the elderly and see if they could give you a blank one. I'm sure they wouldn't mind helping you out.

Both the other suggestions are good but you can also call to your nearest social work department and they will be happy to give you a copyof a careplan form once you explain you are astudent. Best of luck with the case study I'm in my last 6 months of the degree in social care and it's been tough!!
