Most pilots when they are traveling for the company on a trip they usually are a a per diem rate from their employer (this is where the get a certain amount of money per day allotted to what they can spend on their meals). On other occasions the company will just reimburse the receipts from the pilots for what they spend while on a trip, you don't really find that in the airlines, mostly on the corporate side that happens.

That depends upon the hotel. Many hotels include breakfast for all guests.
Commercial pilots normally have their bills covered by their employer, just like any business traveler. Private pilots pay like everyone else.

As a crewmember, I can tell you that no, we do no not get "free" meals from the hotels or restaurants. We are paid a small per diem for every hour we are away from home (in addition to our regular salary). At my airline, it is 2 dollars per hour - so if I am gone from home for 48 hours I would receive 96 dollars to help offset the cost of meal expenses. In addition, many of the hotels offer crewmembers discounts on their food service.

sometimes yes, usually no. The airlines negotiate with hotels for the best rates, and frequntly the "perks" are eliminated as part of the bargaining.
