Does anyone know the anthem blue cross blue shield of ohio billing address? I lost my billing envelope.?

The Anthem website does not have the address listed anywhere! I need to send in my monthly bill for my health insurance but I've lost the envelope that comes with the bill.

call the number on the back of your insurance card and they will be able to tell you where the billing address it.

Did you look on their website? They may have a toll-free number on the website you can call for the address.
