There are two meanings to that::

To suck "verb": chupar/mamar (very vulgar though)
You suck: tu chupas

Example:
The baby is sucking his thumb-el bebe se chupa el dedo
They baby is sucking my breast-El bebe esta mamando mi pecho.

or

No sirves/eres un perdedor (You suck/you are a loser)

"Sofia", has the best answer. She understands both the literal question (to suck, both usages) and the figurative one (slang for you are worthless). There might be some more graphic or perhaps lewd ways of saying "you suck" in various parts of the Spanish speaking world, but "no sirve" is a good general way to express the idea.
