If you say it to somebody it is "toma nota" or "anota" or "apunta".

If it is in general like I have to take notes.
Tengo que tomar nota.
Tengo que anotar.
or
Tengo que tomar anotaciones.
Tengo que tomar apuntes

You can use any of them.

(You'd have to conjugate 'tomar' to fit whoever you were talking about. Ex. "I take notes" would be "Tomo apuntes."
"We take notes" would be "Tomamos apuntes.")
