Your answer is found in the first two verses of Revelation chapter one.
Rev 1:1,2 "The Revelation of Jesus Christ, which God gave unto him, to shew unto his servants things which must shortly come to pass; and he sent and signified it by his angel unto his servant John: Who bare record of the word of God, and of the testimony of Jesus Christ, and of all things that he saw."
Revelation means to reveal. John revealed that which was shown to him.

John was the last living disciple, and probably the only one to die of old age. He was exiled to the Isle of Patmos, a rock in the Mediterranean Sea. While there, he had the visions that became the book of Revelation.
He also wrote the Gospel of John, and The short letters 1st, 2nd and 3rd John.
