You can believe in islam as aconcept . like you say i believe that islam is the way to achieving peace , equality etc. but not practice islam as your chosen religion.
unless you act upon your conviction, you are not a muslim. to become a muslim you have to believe in Islam as the last message from Allah. you have to bear witness that Allah is the only god, and that Mohammad ( peace be upon him) is the last prophet and messanger from Allah.
the five pillars of Islam are .
Bearing witness that there is no god but Allah , Mohammad is his last messanger.
2. performing Salah ( praying to Allah on certain specified times each day)
3. giving 2.5 % of your wealth annually to the poor ( Zakah )
4. fasting the month of ramadan every year
5. performing Hajj ( pilgrimage to Makkah ) once in a life time if your able physically and financially.

They claim that they are Muslims, those who are believers, practicing or following what Islam Religion is teaching. So they say you are a Muslim if you follow Islam. But sad to say, these people are misguided. Why? Because you cannot claim yourself to be a Muslim (Muslim is an Arabic word by definition means righteous man). Nobody on this Earth can claim that he is righteous or a Muslim. Only YHWH Elohim can say or has the final say if you are righteous or not, until He had awarded you an eternal life thats the time you'll know that you are righteous because He had said so. Claiming yourself to be righteous or Muslim is self exhaltation, pride, which only Satan is the promoter of it. He is the first one who exhalted himself and pride. Nobody should claim they are righteous for they will be embarass in the end. We should humble ourselves.

Well, this is a good question. Muslim is by belief in Islam.
A true believer is one who follows and submits himself to Islamic Monotheism (belief in One God, and that Moses, Jesus, Muhammad were all the messengers of God, and Muhammad being the final messenger).
This is done by taking the shahadah
simply saying
Laillaha illah Muhammadur-rasullah
There is no deity worthy of worship besides God, and the Muhammad is the last and final messenger of God.
By professing that, and truly believing with all sincerity then you are a Muslim.

I'm not sure. Because I'm not Muslim I can't say for certain, but I'm pretty sure Islam is the belief, and the adjective used to describe those believers is Muslim.
