I had a grown dog shipped from Iowa to California. I think it was more stressful to us than the dog. We went to Iowa and lined his carrier with papers and a blanket, a couple toys, and food and water. And my old vet gave me doggie downers. When the dog came off the plane 9 hours later, the dog was up and happy and glad to see us. I'll never know whether he slept or not, but he arrived just fine.
And airlines have rules about temperature conditions and have a special cargo space for animals.

we've never had any problems shipping a puppy or receiving a puppy via air travel.
Yes they might be a little scared when you pick them up and take them outside (they won't let you take them out of the crate in the airport) to go to the bathroom. They snap out of it pretty quick.
Please tell me it's not a pug, doberman or puggle or a so called miniature beagle!

it will probably be quite stressful on the dog, i have brought my jack russel shih tzu with me on a plane where she went under the seat which was less stressful for her... if it is at all possible i recommend that you go and pick up the puppy, it will lower the stress levels. not too mention the fact that afterwards the puppy has to get used to new people and a new home

Yes he will most likely be scared, it also can get very cold, so I recomend putting a blanket in the cage. Also he might be bumped around a little bit. So it might wimper. I recomend if you can finding a way to take him up with you personally.

The vet can sell you some pet medication (maybe sleeping pills) to help calm your dog before and during the flight. I did this once when I flew with my dog. When I picked her up in the luggage claim area, she was still groggy, but happy to see me.
