He is a quiet, polite boy who has been busted for riding his bike with friends on busy, dangerous streets. He has been hiding his dirty underware from his parents. He lives in a marginal neighborhood in Costa Rica. His parents have grounded him. He has not pooped his pants for 6 or 7 years until now.

I had the same problem with my stepson. He's not Latin and he is not impoverished. The story sounds much the same so here we go.

At first we thought that he had a problem with personal hygiene. I started making him wash his underpants and then he started hiding them. This had been going on for 2 years. Then earlier this summer we were at summer camp and he pooped in his pants. He was almost 10 and just pooped in his pants. I didn't realize what had happened until I was washing his clothes.

I called his Mom and she said that this had happened before. She took him to the pediatrician and they said it was a control issue and to ignore it. After the 2nd incident, I realized I couldn't ignore it.

I took him our pediatrician who explained that there is a medical condition called encopresis. Basically, my stepson has had irregular bowel issues since I potty trained him. He had been holding it for years and as a result, his body has lost the ability to know when it had to go.

We took him to get a x-ray on his bowels and found out that he had an abnormal amount of stool in his intestines. We then took him to the pediatric gastrointernologist who confirmed the diagnosis.

This is the prescribed treatment:

1. Make sure he is getting plenty of fiber each day 20 grams per day. Calculate the amount per food at www.metamucil.com.

2. Give him 2 tbls of milk of magnesia once a day for 4 weeks.
Generally at night so he can do his business at home.

3. Re-potty train the child. Make him go and try to poop 2 times a day. Let him read a book on the pot, play with a toy whatever it takes so he stays there.

4. After the month, drop the dosage of milk of magnesia to once a day 1 TBS.

5. Most importantly, let the child know that this is a medical issue not his fault and we will work on it together.

Now, my kid isn't afraid to go poop. He likes the orange flavored milk of magnesia and eats a high fiber diet each day. He still has skid marks in his pants but he has control of his bowels and hasn't had an accident since we went to see the doctor.

Good luck, I know how frustrating it can be.

P.S. We also took him to see a psychologist to insure that there wasn't any sexual abuse going on as well.

I wouldn't get in a flat spin and say that the boy has been sexually assaulted.

I would however go to the school and find out if there has been anything unusual happening that i maybe don't know about.

My friends son started doing the same thing and it ended up being a bully at school.

The child is already trying to hide the problem from his parents. Which tells me he can't control it, and it very embarrassed.
Grounding isn't going to help.

He needs loving support, and council.

First of all let me say that this sounds quite fake, made-up, that is a complete lie. Simply because you felt it necessary to say he was Latin, and what he does for fun, sounds to much like a soap opera. For the sake of argument, lets say it is true, then there would have to be two things happen. The first would be to call children services because someone, somewhere is abusing the boy and more than likely it is sexual abuse. The second would be for the boy to receive some counseling.

Sounds like he is super scared of something, possibly something he did and the parents have not yet found out about. He may also be afraid that someone is going to severely punish him for something. Possibly someone is threatening him not to tell on them for being along for the ride. It doesn't make much sense for him to start doing this for no reason. However, he may be doing in protest of being grounded, it may be a way to distract his parents from the real issue.

Sounds like he needs a counselor of some type. Can the parents get their doctor to refer one?

Pretty clear something is seriously, emotionally upsetting in his life.
