I know of two organizations:

www.operationpaperback.org - The site (which is part of the America Supports You network) requires you to complete a brief application. There is no fee for joining and the administrator will email you a list of soldiers' addresses to send books to.

www.booksforsoldiers.com - This site requires you to complete an application and provide a $12 application fee before you are permitted to access the message boards where soldiers post their book requests.
