Well my uncle was part of Dr. Pepper back in the 90's and this is straight from the horse's mouth...
It is carbonated Prune Juice mostly-
And you thought your grandma was crazy for eating those things... I love Dr. Pepper

I love this question:
I used to work in contract sales for a cleaning company that serviced the main Dr. Pepper bottling plant in the South.
One day I ate a whole 1lb. bag of corn chips, pretty much killing my ability to taste corn or any derivitive for a period of time. (Like a few hours, you can try this yourself.) I then drank a Dr. Pepper. Because I couldn't taste the corn syrup, it tasted like nuts. Nuts.
I called our client to see if I could come in for a tour, under the pretense that I was reviewing the work the cleaners were doing. The next day, I ate a whole bag of almonds and corn chips and drank the doctor pepper. It tasted like rum.
When I went on the tour, I asked what the flavor was. THey said that it is a closely guarded secret, but that it is not prune juice, of which there is none in Dr. Pepper, and cherry is not a key ingredient outside of Cherry Vanilla Dr. Pepper.
I told them of my experiment and asked them if Rum and Amaretto were ingredients. The woman said that yes, those flavors were included, but there are more than 200 other ingredients as well.

I just watched a program on The History Channel about Dr. Pepper. The ingredients that give it that distinctive taste is a closely guarded secret, not available to the public.

To me the flavor is almost like Cherry Coke
Carbonated Water
High Fructose Corn Syrup and/or Sugar
Caramel Color
Phosphoric Acid
Artificial and Natural Flavors
Sodium Benzoate (Preservative)
Caffeine

Geeze some people. There is no prune juice in Dr. Pepper. Go to the link and look at the FAQ's.

Supposedly there are 23 ingredients that make up the taste, but mostly it tastes like root beer with cherry added to it.

The urban legend is that it contains prune juice... which it doesn't, see link below. Besides it doesn't even taste like prune juice. I've tried prune juice before.
To me it taste slightly like black cherry soda... although the link below says it does not contain cherry either.
However, when I've tried black cherry soda, various brands, it reminds me of Dr. Pepper.
