It would depend on how that person accepts what is being taught. If it makes sense, a short time, maybe even one time. If it doesn't make sense, it could take a long time, maybe even hundreds of affirmations.

Depends on the person. Also depends greatly on the affirmation. If it's simple and easy, not long. If it's more complex or "unbelievable", for most people it will take more times--however, after having a few of those actually work, you'll find it much easier to do it again later with even harder things.

Also, I'm a visual person, so repeating stuff doesn't work for me--I forget what I hear. I write things down and keep them in a place where I'll see them often instead.

ETA: To be honest, my preferred method is to simply seed the subconscious and forget about it. Works even better in my experience.

If it's irrational, I recommend re-affirming it every Sunday morning. Maybe more. Or just giving it up.
