Yes, but you don't have to say anything. Actions do speak louder than words. Just talk with that person like nothing ever happened. I'm not saying to ignore what happened and not to talk about it. Just ease into it. After someone has hurt you, you are the most vulnerable to be hurt again. And if this person is someone you care about and who cares about you, then that person is probably hurt as well. If not, then maybe they don't know what they did. this is when you sit down and talk with them about it and let them know what they did. That's when they will apologize and all will be alright.

It would depend on the circumstances.

If the other person doesn't feel the need to be forgiven (i.e. doesn't think s/he did anything that would need forgiving) it would be EXTREMELY rude to do this.
