Only put your name on if you're a business and looking for free advertising - otherwise, why would you want to - it's not going to fast-track you to heaven?

I give mostly Anonymous because if not, I start getting letters from every one who wants a handout. Some are plain old bum's.

You can tell the charity not to print your name, if you want. It's not self-promoting -- I think it encourages others to give. "Hey, look, they gave, I should, too." You can also tell them not to give your name to other organizations, if you're afraid you'll just get more solicitations. You can even tell them not to mail to you, or only to mail once a year, if you don't want them spending the money on mailings. It's all up to you.

Depends on if you want the tax deduction. I don't think it's wrong to do either, but I certainly don't think it makes you a martyr if you abstain from giving your name.

It has nothing to do with Ettiquette - you can ask Miss Manners if you want verification on that (Judith Martin).

If you don't want your name used, say so. It's up to you.
