A man and his wife are driving, the car suddenly breaks down and he tells his wife that he's going for help and she's not to open the door for anyone.
When he returns he finds his wife dead and a total stranger in the car...

The women was pregnant and gave birth to a baby and died during birth. The baby was the stranger.

You said she never opened the doors and the car windows were not broken -- were any of the car windows down? He told her not to open the door for anyone, but he never told her not to open the window for anyone. I'm guessing the stranger walked up and tapped on the window, so she rolled the window down and the stranger was able to get in and kill her.
