Here is what works for mine:
All at once......
3 aspirin
2 Tylenol
2 Sudaffed decongestants
1-2 cups of coffee or tea or cola
Ice pack on the back of my neck, forehead, around my ears.
This works better then the prescription medicine my doctor gave me.

There could be MANY causes for a severe headache. Best thing to do would be to talk to your doctor about it. For me, personally, I get headaches from dehydration, certain smells, toxic fumes (i.e. riding in a car and smelling exhaust), motion sickness, my monthy cycle, low blood sugar, muscle tension and eye strain. Each remedy is different depending on what caused it. Dehydration is the easiest to fix by prevention - staying hydrated, and if I do become dehydrated, drinking plenty of fluids - not just water. Also, low blood sugar - I usually eat some protein - not more sugar as this will make it only worse! For muscle tension, sometimes sleep is the best. I have taken advil, which helps a little. For really bad headaches (I am talking the kind that throb and make you dizzy and nauseous) I would take Excedrine Migrane, which really helped, but would make me very jittery. I also found out that the Excedrine was effecting my liver and caused my liver enzymes to spike (aka liver damage) and my doctor told me not to ever take Excedrine again, nor aspirin) Sometimes cool compression helps with tension, or gently biting on a pencil or finger to help relax the jaw. If it's a smell or toxic fumes, there is pretty much nothing I can do except wait. I get out of the situation as fast as possible - but these headaches can be horrible as they might take at least a day and night to recover, or sometimes only an hour. If you are a woman, hormones can induce headaches, as they do for me right before my period. Usually, I try and take advil EARLY. My doctor told me that if I wait until the pain is too great, the medicine won't be as effective. Again, talk to your doctor. There are probably hundreds of reasons one gets a headache - ranging from simple causes to severe. Any pain is a sign that there is something amis, especially recouring symptoms. Good luck. Sorry you are having headaches - they really stink! :-(

When I get a migraine (horrible pounding pain, usually one-sided, with nausea and dizziness) I take an Excedrin Migraine at the first sign. This usually takes care of the headache portion but not the nausea and dizziness, which some people don't get. So if you just have a migraine, Excedrin Migraine 2 tabs could work for you. Don't take more than 2 in 24 hours (according to directions). If you're not sure if your headache is a migraine or a different type it's best to see your doctor, who can pinpoint exactly which type you have and medicate you properly.

I have read that taking alternating hot and cold showers helps break the vasospasm. Try it.
