Chocolate can potentially be poisonous to dogs, however it's not as bad as most people think. It depends on the type of chocolate - milk being the least dangerous and bakers or the darker the more dangerous, that said it's still not as bad as common thought would make it seem.
Grapes - no body really knows if grapes are dangerous or not, they are saying that they are because there have been many reported deaths from dogs eating raisins, and as they don't know what it is about the raisin that is poison, they suggest avoiding grapes as well, ( I personally worked with 2 dogs from the same family that died from eating raisins - this owner was key in making the public aware of this danger)
If your wanting to give your dog people food - stick to healthy cooked veggies and fruits other than grapes. Onions again theoretically are dangerous, but I've never seen a dog sick nor die from eating something with onion in it or onion flavoring but it's best to avoid it.
Check out the Poison Control website for a very detailed list of toxins.

Onions are a definite poison to dogs, as is chocolate. But then, chocolate in sufficient quantity is also poisonous to humans.

Yes, grapes can kill a dog if they eat enough of them. So can chocolate, onions, garlic, mushrooms, macadamia nuts, and the list below. There are lots of plants and flowers that can do so as well. There are lots of sites w/ this info if you do a Google/Yahoo search for them.
Other potential dangers:
Pear pips, the kernels of plums, peaches and apricots, apple core pips (contain cyanogenic glycosides resulting in cyanide posioning)
Potato peelings and green looking potatoes
Rhubarb leaves
Mouldy/spoiled foods
Alcohol
Yeast dough

Here are some healthy people foods for dogs:

PLease do not listen to anyone telling you that grapes are not harmful. THEY ARE. These people are going to find out that grapes do have long term effects and these effects don't generally manifest themselves until the dog reaches old age. RENAL FAILURE.
Go the the ASCPA Animal Control Poison Center web site and you can research anything you want with regards to what is harmful to your pets.
PEOPLE...JUST BECAUSE IT IS NOT CAUSING YOUR DOG ANY HEALTH PROBLEMS NOW DOESN'T MEAN IT WON'T IN THE FUTURE!!!

Ok they dont poison a dog, they just make them feel sick. And my dog eats green grapes, he loves them. But not purple, and Im not sure why.
But you really shouldnt feed a dog anything but dog food and treats. Table food, bones from chicken ect, will make your dog constipated and chocolate will also make him feel sick. So stick with dog food and dog treats for your dog.
