I think that the ethical nature of the transaction depends entirely on the intentions and emotions of the parties involved. If the client feels compelled to provide the favors (because s/he really needs the legal help and has no other way to pay and you refuse to offer pro bono assistance) then I believe the barter is entirely unethical (your question seems to imply s/he may not be 100% ok with the arrangement).
However, if s/he is comfortable with it and/or if it is his/her profession, then it would be ethical. If this is the case, you might consider writing a contract that stipulates precisely what services will be performed by both parties and indicates that the agreement was entered into without coercion.
I would be very careful with this whole thing though, as the person might very well be able to nail you with this later and you wouldn't want to be disbarred.

Yes on both cases.
Trading legal advice for sexual favors is unethical and reneging on a promise, while not illegal, simply tarnishes your character.
Eventually, your reputation will keep away honest business and you'll be left to deal with more gray-area clients or worse, organized crime.
Then again, you might be OK with that.
