Beyond the yield point, deformation begins to occur in a material, and often leads to "necking", or localized reduction in diameter.

Stress is calculated as Force / Area. The area is a cross-section of the material.

If the nominal stress you speak of is also known as engineering stress, then it is calculated assuming the area at any time during deformation is equal to the initial area.

The actual stress takes into account the area of the cross section at the given point in time.

The relationship between the two is:

σT = σ * (1 + ε)
or
True Stress = Engineering Stress * (1 + Strain)
