Hi,

I have used Pisa for several years.
This is a small airport with all the major car rentals.
But if you are used to pick up a car on airports it's not a big deal anywhere in Italy.
I have used both Ciampino and Fiumicino near Rome.
If you just don't try to bring the car to the center of Rome it's quite easy to pick up and deliver your car there as well.

Italian airports are notorious for their chaotic "organization" and the customer unfriendly attitude of the personnel.
Milan's Malpensa is the worst, so at least try to avoid arriving there.

Rome Fiumicino is the best, then you can go to Naples, Sorrento, Capri , Pisa, Florence, Venice, Milan, & Monaco.

Don't "fly into" any airports!
Insist that your pilot "LAND" at an airport!
