Generally, you will find that the dental profession is 'fee for service'. This will mean that there is a charge for each service performed... examination from the dentist, xrays, and prophy by the hygienist. Unlike a medical office where you are charged for an office visit (just for coming in) and anything else that is done, bandages used, etc.

Depends on where you are located. Some hygienists can practice without the direct supervision of a licensed dentist.

normally it comes with the treatment with private healthcare but it costs me £12 to get my teeth cleaned by a hygenist.

Depends if you are on NHS, Denplan, or private - I am private, so it costs me about £38. If you are on NHS or Denplan, ask your dentist receptionist.
