Well, if you had back, neck and shoulder injuries that extensive, you are going to have a certain amount of pain for the rest of your life. I guess you've already figured that part out. There isn't a magic formula to make it go away completely, and you are going to have some days better than others. You have to have a fairly large bag of tricks to try to help get through the days and function. I broke my neck in a car wreck myself, years ago, and have residual nerve damage to my left arm and neck as a result. When the weather and I get cold, I have more problems than at the height of summer, and weather changes can trigger them as well. Like you I am best friends with the heating pad, and I often smell of the "granny" rubs like Deep Heat in the winter.
You can safely take up to 800mg of Ibuprofen, with food or milk, up to three times a day- but not every day for days on end. You can also take Naproxen Sodium, sold in generic form and as Aleve. It's an anti-inflammatory and pain reliever designed for arthitic pain, and works pretty well. There are several other anti-inflammatory drugs now available over the counter, just ask the pharmacist to help you find them. Now they even have the chemical heating pads available, so you can cut the electrical cord and be able to leave the house.
I've never had much relief with ice,to be honest with you. When my muscles really get tight, I get a nasty headache and my left arm tends to go numb. That's when I head to the sauna and whirlpool at the gym. I find a while in the heat there really helps loosen things up for me. It doesn't make it all better, by any means, but I get enough relief to be able to rejoin the living world. Working out at the gym helps as well, believe it or not. Just keeping the muscles in good tone and sometimes I can even work out some of the soreness.
There really isn't any surgery that will offer you and I any kind of relief. They can't repair the damage we did, so theres nothing they can do. The most they can offer is physical therapy, and you can do that yourself and save the expense. I also recommend finding a good Doctor of Osteopathy to treat you. They do manipulations along the same lines of a chiropracter, but can also give you medicine when needed. They prefer to try non-medical treatments and to work with the body first, but they certainly aren't opposed to pain relief when it's required.
Make sure you have a good mattress, as you can really feel more miserable trying to sleep on a soft saggy one. When my shoulder and arm are really in bad shape, I sleep sitting up, either propped in bed or in an easy chair- just to keep the pressure off it. And for some things, I just ask for the help when I need it. Get a nice massage shower head for the shower, as I find a nice hot shower with that can also help a lot. So can a nice hot soak in the tub- if you don't have or want a shower.
As I said, there isn't any one thing that will work best or all the time. It's just a matter of experimenting to learn what works for you and what doesn't. Some days, you just have to do the best you can and keep plugging away. And who knows, maybe someday they will figure out a better fix.

Go see a Doctor that can and will help you, there is no reason they shouldn't, you should have some imaging done as well to find out what is causing this pain and if indeed you do need surgical intervention, and depending on what it is and the severity of it will determine weather or not there are surgical options.
Hope this helps!

This is not the place to get answers-you need to see a doctor ASAP. You've been guessing for years now since your accident. It's high time you got real answers from professionals. How do you expect doctors logged on to help you if they cannot examine you?

I agree you should get a ortho/neuro consult. In the meantime, try a jacuzzi, steamroom. and a big fat joint for the pain. Maybe consider aquatic rehab? Good Luck !
