It refers to the bodys release of adrenelin in a fear/stress situation. The adrenelin prepares the body to defend itself physically (fight) if need be or to run away (flight.)

Fight or Flight is the sense on whether or not to run. It was developed by instinct back in the times of cavemen. The men would be out hunting, and when they found an animal that could hurt them, their instinct to them to either fight the animal or run away.

The bodies response to a threatening situation, a choice. Either hit him back if he hits you or "run like hell". Adrenalin (a hormone from the adrenals on top of the kidneys) is released at the perception of a threat which primes the body energy-wise to either fight or to flee.
