In the LDS church, how is someone "saved"? And what happens after they die (for eternity)?

As far as "saved" we don't believe in the trandtional sense of that the way "Christians" do....you should read "Have You Been Saved" on LDS.org in the conference talks...it is just a short talk given by one of the apostles and shows the LDS views of "saved".
After everything (death, resurrection, millenium, judgement) We believe there are 3 kingdoms one can attain glory in: Celestial, Terrestrial and Telestial. Attaining glory in one of them is contingent upon covenants made and righteousness in this life. Celestial is the highest one can attain, where Christ and God the Father dwell and where there is chance for progression for a married couple and the family unit. You can read about the "criteria" in section 76 in the Doctrine & Covenants.
Also, a good place to look on topics is at LDS.org in the conference talks...there have been talks about pretty much every subject.

We believe that in order to be saved one must believe on the Saviour, Jesus Christ, and accept his atonement. Jesus suffered and died so that we could be forgiven of our sins (we have all sinned) and have eternal life. So there are four steps (called the first principles of the gospel).

1. Faith in the Lord Jesus Christ
2. Repentance
3. Baptism
4. Gift of the Holy Ghost.

Unlike other Christians, we believe that although it is ultmately the grace of Jesus which saves us, not our own works, our salvation is a two-way covenant. When we choose to follow him we are expected to do just that - to live as He would have us do. We therefore try to live righteous, worthy and compassionate lives, and turn away from sin and evil. We know that we can repent when we do wrong, but if we are truly Christians we will pattern our lives after Christs.

After we die we go to the spirit world for a time, and then there are three places we might go. One kingdom equates to "heaven", where good people who have lived generally decent lives can go. Another kingdom equates to hell, where those who have sought only evil and the suffering of others are forever cut off from God. However, we also believe that we are children of God and ultimately can continue to progress throughout eternity, becoming more like him. Those who have received the necessary ordinances go to the "Celestial Kingdom" where they can be united as families in the presence of God for eternity. Kind-of like an even-better heaven.

Pinkadot's answer was really good. Let me add to hers by saying we being saved as process more than a one time event. Yes the ordinances such as baptism are important, but then there's a whole lot of enduring to the end after they are all done too.

Mormon theology is incredibly flawed. There are so many inconsistencies in the teachings of the Mormon religion that it is odd any thinking person would follow it. Do your research before you join the LDS church.
As a community they are strong, very helpful to each other and dedicated to their beliefs...even if they are odd.
Being saved??? I think we are all going to the same place when we die....____________________you fill in the blank as to where that may be.

... lets see, you have to first be baptized, (one of the saving ordinances.) second you have to live your life as Jesus would have you live it. if you want to obtain the highest kingdom of god, the celestial kingdom, you have to be married in one of our holy temples, and keep all of the covenants you make there.

(there might be more to it than this but you cant really expect a teenager to know everything about his religion, i am always learing more about it myself, there is lots to learn!)
