Because they're trying to excuse their lack of manners by prefacing what they say... Either say what you have to say and take responsibility for how people react to it, or keep your mouth shut until you figure out whether or not you really need to say anything at all. I think *some* people really mean "I want to tell you something, and I say this with only the best intentions." Really, what it comes down to is taking responsibility for the words that come out of your mouth and doing your best to ensure your full meaning is communicated clearly. Communication is a two-way street, though -- some people just live to be offended. You really can't do much about that.

Well, you want to make sure that you create as few enemies as possible.
Life is hard enough as it is.
And people say "no offense but..." to give the message that they really don't want to offend you on purpose, however, the thing that they are about to say would probably offend you. People want to say that they are telling you this in good faith and that they hope that after this you'd still be able to have a good relationship,
No one says "hold back the truth" but there are many ways to do that in a civil manner, no matter how offensive you may get.

I think that when someone says, "no offense," they are trying to say that they don't mean it in a negative way, like don't take it personally.
Stupid is what stupid does.
Stupid is an offensive word.
As one person to another I say that I hope that we do not intentionally try to hurt one another. I wish that no one would but I know that people do just to be mean just because they can.
It is sad.
We have other more important things to be working on.
Peace, joy, and love.

Just to soften the blow of what comes next. It's a modern form of being polite. With all the obnoxious and deviant people out there today, I could not be offended by this. And, I think people that say this are trying to speak their truth, just with a little grace.

People say "no offense" because they want to tell you the truth (in their opinion) and try not to hurt your feelings at the same time... but I agree with you... it's stupid to say "no offense" when that person knows that what ever he/she just said is offensive. Hope I helped! :-) :-0

It's called political correctness, it's a big thing with the Left. I hate it and don't care who I offend but then again I'm not one of these users who go out of their wat (as you say) to cause trouble by deliberately been racist/sexist/hateful.
But when people stick on "no offense" its usually to show they are asking a question that is about race, sex or other cultures or other religions BUT is merely been asked because he/she is curious and not because they want to cause trouble on Yahoo! Answers

I think they may have difficulty asking a certain question, and are trying to be nice. Some on here purposely seek to offend. That's the difference.

We live in a politically correct society but also have the need to air our views. Its just to forewarn those who may get offended.
