If you are referring to registering as a student with a disability at your school, you will need to contact the Office of Disability Services at your school. Each college or university has different requirements. They will all however, require official documentation of all physical, psychological and learning disabilites for which you want to apply for special accommodations.

start with your family doctor and he/she will refer you to a specilist in whatever is ailing you.Than you call up cpp (in canada) and take it from there.
next will be visits to cpp's doctors who will either crtify you for temp, Perminent or no disability.
