Implementation is carring out the actions to reach the goal you have set for your problem or diagnosis. So you have the problem then goal then action list and then you implement the actions. After this you evaluate

This is not in reference to nursing but will give you an idea:
Implementation is basically the start of something e.g. a new Software being incorporated in an industry or the start of a new highway.
Actions are the procedures or processes one has to follow in order to complete a task.

Action is an operation or deed.

you can start implementing things ie things must be done this way or that way, or you can just get on with it.
put another way
I have an idea, right set up a committee then a steering committee
or I have an idea and get on with it.
