I already know that the blood gets oxygen from the lungs and pumps back into the heart, but where does the nutrients come from?

Nutrients come from food. This is a very involved, intricate and beautiful process.

In short, during the digestive process, the food is broken down by the teeth into small pieces, and then swallowed into the stomach. From there, it goes into the small intestines, where enzymes break the food down into the smallest pieces to be absorbed through the intestinal wall and into the blood stream.

All carbohydrates are broken down into single molecules of glucose like monosaccharides; protein is broken down into amino acids; and fats are broken down into fatty acids and glycerol; vitamins, minerals and phytochemicals are released and absorbed into the blood stream also.

When these nutrients get into the blood, they circulate throughout the body, wherever the blood goes. By osmosis, the nutrients move through the walls of the blood vessels and into the interstitial space around the cells and bath the cells. Whatever the cells need, they take from the interstitial space.
