All of the above, plus you need to practice a language about once per week to keep from loosing it, once per day is better.

Regular exposure to the language. If you go without speaking, listening to, reading or writing it for a period of time, you will forget. Even though I interact in Spanish for some time almost every weekend, I find that my Spanish isn't quite as good as it was when I was using it every day.

Definitely repetition and practice.
When I am learning a new language I use my vocabulary and verbs whenever I see or do those things.
Like if you'e making breakfast, think of the words for all the food and utensils you are using and form sentences of your actions. For Spanish that could be something like:
Estoy pelando naranjas. (I am peeling oranges)
