If you're leaving the state, you'll usually need a record of current vaccinations, including rabies, and a health certificate issued by your vet.

Your veterinarian will be able to provide you with all the documents you need. Your dog will have to have an exam to get its health certificate.

Rabies is now given as early as 12 weeks.

I assume you are flying with your baby which I have done many times. The dog has to be up to date on all shots and Rabies. You need paper proof as well as a Health Certificate. One the plane you need a regulation dog carrier, I recommend the soft mesh ones, not the hard plastic. If you are driving you will need the same documentation from your Vet. Have a good time.

You need more info. in your question. Are you going to be traveling on a plane? Are you going to be crossing a border? Are you going from state to state?---- If you are going to be traveling on a plane, you need a Veterinarian certificate. This will certify your dog has all her vaccines. This might be a problem. Reason being your dog is to young for a rabbie shot. It is a good idea to wait at least six months for this vaccine. If you do give him this shot before the six months, you can damage your dogs health.
Since your dog is of a small breed, it is not a good idea to give her all vaccinations at once. A good vet. will tell you this. If you are going on a plane you will also need to have your dog in a travel container. Since your dog is of a small breed a travel bag would probably be best, that way you can take her with you on board the plane. You will also have to pay about $80.00 for her.
--Remember you can have your vet. write on the certificate your dog is to young to have her rabbie vaccine. Downfall to this is that on your way back, customs may force you to have this vaccine administered to your dog! Otherwise your dog will not boarder the plane. Good Luck!!

That depends.. going out of town from where to where.. If from East Moline, Indiana to West Moline, Indiana, you will need nothing unless you plan to BOARD the pet in West Moline. and if you do.. you will need the current vaccination papers to include the bordatella shot.

If you plan to go out of the country.. then be careful.. If it is to England, they will quarantine the pet for some weeks.. if t other places, you need to get a veterinarian certificate of good health for the pet 72 hours or less prior to deparature.
