If the dog is a puppy, then you can start with puppy pads. Usually you place them in the kitchen and then start moving the pad closer and closer to the door. Also, take them out as much as possible so they will get the idea.

Looks like the majority of answers are crate training. You didn't say what size or breed. Or male of female. Very simply I use positive reinforcement. Praise the good (going outside) by lots love praise and sometimes a treat works too. If your dog has an accident or goes in the house. Be angry at the mess..NOT THE DOG. The dog needs to learn what makes you happy and what does not. Strangely, it is like teaching your dog a trick..Think about it. You teach a dog to "sit", and reward him/her..you certainly don't get angry if he/she doesn't sit when told!! House training works the same way...it is like teaching a new trick to your dog!!
If your dog has been going in the house, you have to get all the urine smell out of whatever your dog has gone on. Nature's Miracle works well on everything. You can get it at most pet stores, and directions are on the bottle.

Crate train the dog. Get a crate slightly larger than the dog just enough for the dog to turn around in. Keep the dog in the crate when you are sleeping, not home or not able to watch the dog. Take the dog out immediately when you take him out of the crate. Take the dog out 15 minutes after eating. Take the dog out after playing. If you are watching the dog and you see him start to circle and sniff around then take him out. If he starts to have and accident in the house take him outside tell him firmly "no potty outside" as you take him outside. Don't yell at dog. Don't hit dog. Don't rub dog's nose in it's mess. If the dog has an accident and you do not witness it unfortunately there is nothing you can do about it. Good luck.

You need to use a crate. When the dog comes out of the crate they go immediately outside. While the dog is walking around you monitor how much water they've had and watch if their sniffing around. Then before you put them back in the crate bring them outside again. You must be consistent.
I have three labs, 12 yrs, 1 yr and a 5 month old puppy....and it's worked for all of them. My dogs have never had a mistake!!

Buy an appropriately sized crate for your dog. It should be big enough for him/her to turn around in but not so big he/she could potty at one end and still lay comfortably at the other end. Keep him/her in the crate while you are away from home and at night while you're sleeping.
You'll have to go out with your dog EVERY time and watch for when he/she "goes." Make sure to take a couple treats out with you. When he/she does their busines, give a treat and give LOTS of praise. Make a big deal of it.
If he/she does go in the house, you can only punish IF you catch them in the act. You tell him/her NO and take them outside.

I've found the best course of action - for my dog - to be buying those housebreaking pads and get the dog to use those. Combined with getting her onto a schedule of going outside and doing her business outside (praise them when they do anything outside).

Crate train him; get him on a potty schedule and stick to it as closely as you can; watch him more closely to see signals that he has to go. All of these will help immensely :o)

if he starts sniffing the carpet or looking like he's going to go, take him outside. if you keep doing this, he'll want you to take him outside and start whining or something

Stick it's face up close to it,tap him/her on it's nose gently wile saying "NO"!& than bring him/her emidiatly outside & say "OUT HERE!" Also, you must watch him/her as much as you can & as soon as it looks like he/she is looking for a spot to poop/pee stop him & say "NO ! OUT SIDE . Bring him/her emidiatly out side. Watch him/her like a hawk.
