I love doing this for someone. It's easy for me and so important to them (I enjoy the expression when they read it the first time).
You say only positive things. Be truthful, don't exaggerate. Short and sweet is okay. Mainly, the reader wants to know that this person isn't a major loser that needs to be avoided.
Here's an example:
My name is so-and-so. I work at BlaBla, and Whatserface is my co-worker.
I see Whatserface throughout the day, as she performs her tasks. She moves with purpose, and is attentive to whatever she's involved in. Her name comes up when various Supervisors need assistance or are in a pinch, as they know she'll apply herself to solving their dilemma. She is always pleasant, uses positive speech, and is a fine representative for this agency.
If there is anything else I can help you with, please call me at "(xxx)xxx-xxxx"

I believe if it there were not some sort of internal conflict regarding this co-worker, you may not have asked the question at all. Be sure this is someone you can truly vouch for because it is also your credibility on the line.
If there is no problem, it should be easy writing about their professionalism, capabilities and whatever makes them outstanding to you.
If in your gut it doesn't feel right, then by all means tell them that you're unable to do write the testimonial on their behalf.

Short, concise, and honest. Even if you have to go with relatively mundane things like "puctual, organized, willing to work as part of a team", rather than really glowing specifics, don't feel you have to exagerate or lie, since this can come back to haunt you if you work in a small, tight knit industry.
