Call dog food companies. Many like Iams,Pedigree,Science Diet, Puppy Chow....all will send you free puppy packs to give out to the new owners. They Always have vaccine records included with them.

Hello, I'm an Iams breeder so if you'd like to email me, I can upload you a copy of the health record that we use and you can download and print it out.

Oh, and to the person that had heartworm prevention listed every 3 weeks - you are wrong about that. heartworm prevention starts at 16 weeks of age and continues throughout life with one pill per month!

i would try the AKC's website, they have all sorts of good things to check out, or you can ask your vet if they have a blank one they can give you

Check out this website...

Hopefully it works. Otherwise, you could probably request one from your vet.
