How to Apply for a Fresh Passport

Application Form

To apply for a passport at Passport Office, Bhopal, the applicant should reside in the districts under the jurisdiction of Passport Office, Bhopal. The applicant is required to fill a fresh passport application form (Form No.1). The application form is available at the Passport Office at a cost of Rs.10/- or it can be downloaded from this site. The duly filled-in application form may be submitted personally or through a representative (having authority letter duly signed by the applicant as per Annexure 'L') or by registered post.

Application Fee

The following all inclusive fee is to be paid along with the application form either in cash or by bank draft drawn in favour of Passport Officer, Bhopal. In case of bank draft, full name of the applicant and application number should be written on the reverse of the bank draft.

Fresh Passport (36 Pages) of 10 years validity Rs. 1000/-
Photographs

Identical photographs in colour of 35 mm X 45 mm showing frontal view of full face against a light background are required. The photographs should be pasted on the application in the boxes provided. Photographs should not be stapled. Photographs taken with coloured or dark glasses and in uniform will not be accepted.

Documents

Proof of residence

(Photocopy of any one of the following is required)

Ration Card
Election Commission Voter Identity Card
Water, Electricity Bills, Telephone bills
Running Bank Account ( Last one year )
Letter from employer on official stationery
Proof of Date of Birth

(Attach photocopy of any one of the following)

Birth certificates issued by Municipal Authorities or district office of the Registrar of Births and Deaths
Matriculation Certificate issued by School Boards
For illiterate persons, affidavit Sworn Before a Magistrate / Notary Stating Nationality and Date of Birth as per specimen in Annexure 'F'
Proof of eligibility for Emigration Check Not Required Stamp (ECNR)

(Attach photocopy of any one of the following)

Degree Certificate Issued By Recognized University
Three Years Income Tax Returns In a Row with actual payment of Income Tax
Three Years Diploma Certificate Issued by Technical Boards
Provisional Certificate with Marks Sheets of All Years / Semesters in case Degree not Awarded.
Certificate of 10+2 qualification from a recognized Central/State Board (If final certificate not available, provisional certificate plus marksheets to be attached)

No Objection Certificate (NOC) (Applicable only to Central or State Government / Public Sector / Statutory body Employees) (As per Annexure 'E')

Please attach NOC in Original With Establishment Number and Date. The date of issue of NOC should be less than six months prior to the date of submission of the application.

Proof of Citizenship (Applicable only if applicant is a citizen of India by Registration or Naturalization)

Photocopy of citizenship document.

Additional Personal Particulars Form (Applicable only if the applicant has not stayed at the present address for the last one year)

Please photocopy the 'Personal Particulars Form' and enclose an additional set (in duplicate with photographs) for each additional place of residence, in case the applicant has not resided continuously at the present address for the last one year.

Note : In case the application is submitted on the counter, all the originals of the documents submitted should be produced for verification. In case of postal applications, photocopies of the documents should be attested by a gazetted officer. The rubber stamp of the attesting authority should clearly indicate the name, designation, office and place.

Tips to fill the Application Form

Please fill in the form legibly and correctly in block letters with expanded initials as per your matriculation / birth certificate. Please put the specimen signatures with ball point pen (not with ink pen) or thumb impression neatly in the boxes provided in the application. All 'Yes' or 'No' columns in the application should be clearly replied to avoid delay and inconvenience.On suppression of material information, penalty applicable will be imposed. Please go through the checklist before submitting the application.

Where do you live? If you live in India, then you can obtain a passport application form at your city's central post office, passport office, or can be simply downloaded.

you must go to the american embassy and apply for your childrens passports if under 21. take all your birthcertificates and your passport. Your children even though the were born out of the us are AMERICANS. Again do not go to INS that process is extremely expensive and takes 3-5 years. As to going to the American Embassy will take if anything 90 days and for under $200.00 each. good luck
