Active voice is when the subject of the sentence (I hope that first you realize that Bertha is the subject in those three sentences) is performing the action. Pasive voice is when the action is performed ONTO the subject of the sentence.
The pasive equivalents to your sentences are:
THE REPORT WAS SHOWN by Bertha to her client.
THE REPORT about her client WAS DISMISSED by Bertha.
THE REPORT WAS WRITTEN by Bertha with her computer.

You also have to learn to recognize direct and indirect objects. That'll make it easier for you to find the instrumental.
Direct object is the noun (or noun phrase) that is acted upon in a sentence AND it is NOT the subject. In all of them, "the report" is the direct object.
Indirect object is the noun (or noun phrase) that gets "the benefit" of the action performed. You only have indirect object in the first one: "to her client".

So far there is no passive voice in your sentences.
Look at the following:

Bertha’s client was shown the report (by her).
Her client was shown the report by Bertha.

The reort about her client was dismissed by Bertha.

The report was written by Bertha on her computer.

I think all of those sentences are active. And all instruments should be the report. Passive should be:
The report was showed by Bertha to her client.
The report about her client was dismissed by Bertha.
The report was written by Bertha (with her computer -- this sounds funny -- I think it should say "using her computer"?).

The only instrumental in this is "with her computer".
All three of the sentences are active voice, there are no passive sentences.
