The best way to improve how you speak in any language is to practice.
Try meeting up with other English learners and spend 20 minutes or even more just making conversation in English. It's not embarassing at all, because no one is an expert and it's a lot of fun too.
I already speak Spanish, but practicing really helped me when I wanted to speak Italian.

For confidence:
1. Relax. It doesn't matter if you make a mistake. Some people that have been speaking english all their lives still don't have proper grammar.
2. Practice. Talk to anyone that will listen. Even inanimate objects.
3. Fake it. Act like you're confident, and you'll start to believe yourself.
4. If you make a mistake, laugh it off.
5. Speak english whenever you can (practice). Like, always use english when amoung friends that know some english.

Taking English classes is the most obvious answer, and though I do not know where you are, I can tell you that these are offered cheaply all over the United States. But it seems as though you already know the basics. Perhaps the best thing for you to do is to spend time around English speakers (say at work or school.) Again, your physical location would serve to limit or expand your options.
If you are in the United States many high schools and colleges (particularly community colleges, two year colleges and the like) offer affordable ESL (English as a second language) courses to the general public. Many private companies do as well, you can find these in the yellow pages or via an internet search.

The very best way is to go and live in an English speaking country for a while. If you did that, then you would be speaking and hearing English every day.
