keep in mind this answer is for heavyweight division only.bob fitzsimmons lost his belt to james jeffries (1899) and 3 yrs later he failed in a bid to regain the title by being stopped by james jeffries again(1902).actually upon rechecking it seems james corbett who lost his title to fitzsimmons in 1897 lost in his title bid with james j jeffries in 1900 so corbett is the first former champion to recieve a title shot and fitzsimmons is the first to recieve a title fight rematch for the heavyweight title.

Good question sir! As far as heavyweights go, I believe that when Jack Dempsey fought Gene Tunney the 2nd time was the first time an ex champ got a second crack at the championship.
