For medicines that require administration on an empty stomach in order to be absorbed properly, patients should choose times of the day that are convenient to them. Before breakfast is ideal for once daily medicines, but other good times include mid-morning, mid-afternoon and last thing at night. Bisphosphonates should not be taken at bedtime.
• The administration of some medicines needs to be timed just before meals or they will not be effective.
• Not complying with these instructions for the occasional dose is unlikely to be a problem, but doing it all the time may mean that the medicine does not work.

Some need to get absorbed more than other medications, and food can mess that process up. Others may make you naseated, and therefore it might be better if you didn't have anything in there to puke up.

because they may make you sick to your stomach and the less you have on your stomach, the less chance you have of being sick
