Hello...I really feel for you in this difficult time...if it helps to know that someone else has been in your shoes, you have my support...I was raised by maternal grandparents and they refused to allow me to use my fathers last name...they eventually adopted me and gave me their last name...well I searched for and located my in another state when I was 34 years old...I hired an attorney who plead my case before a judge and the court allowed me to legally change my last name back to my father's...my father died 5 months after I found him...he was never physically in my life but was and is now spiritually, as you father to you...I hope this helps...blessings to you and your father....If you need someone to chat with...I'm a good listener chivoazul@sbcglobal.net

Go to the court house to do a name change. There may be a form online depending on what state you live in.

Add it by signing it, calling yourself the name, legally you can do a name change by a Deed Poll which is basically a notarised declaration of your intention to use that name as of XYZ date. Good luck God Bless you and your Dad.

I am very sorry about your Dad, I will keep you and your family in my prayers. I am not sure if he would need to adopt you or not. you man want to contact an attorney and ask some questions. And you could just hypenate it Smith-Jones. Good luck and God bless****

legally you have to go to court to get it changed, so make a appointment early because you don't know how long it will take for them you to get a court date

go talk to a lawyer he can help you with this. it will cost $100-$300 dollars to have it done.
