your state should have a medical board or a licensing board where you can ask about stuff like this...find out if they have been disciplined,have ever had their license revoked etc

It may depend on what state you are in. For example, in Georgia, the State Board of Medical Examiners is responsible for licensing doctors.
They have a website where anyone can go to get information about doctors- like when they graduated med school, where they graduated from, if they had ever had any complaints against them reported to the board for which they have been disciplined, has their medical license ever been suspended or revoked, are they board qualified in a special area etc.
So you may want to check and see if the licensing board for your state has a similar website.
Other than that-you could ask the doc or his staff. A patient who is putting his life in a doc's hands has a right to know what kind of doc he is trusting.
Keep in mind that if the doc actually has a license he probably has not done anything too horrible or it would be taken away by the state.
