She does sound like she would be a good candidate, but I would recommend having a consultation with a surgeon for her to best find out if she meets the criteria and learn about the risks and benefits associated with the procedure. Many surgeons' offices offer payment plans, where she could put down a down-payment before the surgery and make monthly payments thereafter. Other offices may offer financing programs through credit companies like CareCredit. I would suggest calling around to some bariatric surgeons in your area to get information, and then schedule a consultation.

I would like to recommend that your friend look into lap-band surgery. A lot of people, myself included, get the gastric bypass because our insurance will pay for it and not lap-band as it is not usually covered. My surgery cost about $27,000.00 and the lap-band would have cost $15,000.00. It is cheaper mostly because there is no hospital stay, but you get almost the same results, just in different time frames.
She will also have to ensure that she does not get pregnant for 2 years after having the surgery. With lap-band if she gets pregnant they can let it out until after the pregnancy.
The problem is that without insurance, if anything goes wrong with the surgery (even with insurance that doesn't cover the surgery) you could end up paying thousands of dollars more, especially if you have to stay in the hospital longer.
I know a lot of people go to Mexico for the surgery, but I don't know anything abou that except that the death rate is much higher in other countries than in the U.S.
One more thing to consider, I was 27 when I had my surgery and the surgeon had some hesitation with me being so young, so your friend being 22 may hinder her.
You need to tell her, and maybe go with her, to go see a center that does the surgery. They should be able to help her with a payment plan and give her options. She will need to be approved for the surgery by her primary care physician though before the surgeon will perform either surgery.

I am a gastric bypass patient, I had the surgery 17 months ago. I was 43 years old and 330 Lbs. Today I am 45 years old and 175 Lbs with no hanging skin. I was in the military for 20 years and gain a lot of weight when I got out- that is why my skin bound back so easily.
However, I was very overweight with High Blood Pressure, Diabetic, Acid Reflux and Gout. I was talking 5 different Blood Pressure pills, Medication for Acid Reflux and Gout plus Diabetic Medication (Metformin). I would suggest that you really have health issue that are life threaten rather then do this for cosmic reason, because it is major surgery.
I am not sure how she could go about having the procedure done without medical insurance unless she can afford to pay for it herself. Although I have heard that Medical coupons her in the State of Washington has paid for many of the procedures because the Patient had life threatening issues.
Another way is to apply for a cosmic visa card Thur Capital One, I know they do some credit cards for some cosmic surgery if your credit is OK. However young lady- as with any other surgery anything could go wrong, that is why if I was you- I would really consider other option.
It was a breeze for me, however for some people its not that easy, some people do not make it. Really give it some soul searching before you make a decision and pray about it.

Your friend's Body Mass Index is 43 so yes she is a good candidate for gastric bypass. Unfortunately this is a very expensive procedure and in the absence of insurance I do not know if you will find someone to perform this operation.
