What is the proper name for a modeling stat sheet... the ones thatn include a photo or two and stats...? Is there a program where you can create one?

Stat also called composit, you can put one or more photos, but the more you put the smaller they will be and of course more difficult to see your features.
They will include all your info.
It might be 5x7, 8x10 or folio (8x10 folded) where you can have one large picture in front, two or three in the center and a large one in the back.

Composite I believe is the term you are looking for. You need about 3 photos with good expresions Close up, mid shot, and full length. Name, height, weight, age, dress size and phone number I believe are the info that it will need.
You can create one in photoshop if you are crafty and artistic and know that program. Or there might be places on the web that can do it for you. Call around to local modeling agencies and see what they require and recommend

you're talking about a portfolio, i would recommend signing with an agency, they do it for you. i've worked for and with ford models for years, you obviously don't know what you're doing, so find someone who does.
