it sounds like you have lost the throw-out bearing on the clutch fork. you need to go to a good shop and have it fixed.

well if this is just happening now u should first let the motor cool down and wait while the car is on for at least 4 minutes. I'm not sure if this is the best advice. You should also go to a Mustang place and if ur in California, go to a Mustang place in Van Nuys.

consider the throw out bearing, bad clutch and plate.
look see if the shift linkage may have shifted, and it is not dis engageing all of the way

you re throw out bearing is gone. this ones gonna cost ya you're gonna have to have a new clutch and throwout bearing hope you have a handy bf the garage is gonna get you for about 2500 bucks

my personal opinion you jest letting out the clutch too early before you shift.i recomed you get that checked by a qualified person or you just need more experience;)
