Theres a balloon type cuff on the end of the Endotracheal tube to hold it in place when a patient is intubated, that needs deflating before you extubate. I extubated on Monday, co-incdence I came across this question!

Most intubation devices have a small balloon filled with air to keep the device in place. We as paramedics have to inflate the balloon then de-inflate it just prior to insertion to make sure there aare no leaks or defects. Prior to extubating the patient we have to remove the air from the balloon so as not to damage their esophagus, etc. Its the same principle used when placing a urinary catheter in a patient...a small balloon holds it in place.

The syringe is used to 'deflate the cuff' an inflatable latex tube used to hold the endotracheal tube in place.
