Urine Gone or Nature's Miracle. Cleaning with anything else will only (as you have found) temporarily hide it. These products both will not hide it but completely remove it by breaking down the crystals in it. I have a brand new house and Urine Gone has saved my carpet from my new puppy!

Cats tend to go outside the litterbox if it gets dirty. It all depends on the cat; with some cats it doesn't have to be very dirty before they will start going elsewhere. When they start doing that, they will pick a spot that they've used before. Even if it has a bit of (indetectible to humans) odor on it, they will find it and use it.
I had to get rid of a favorite parka because of that! They kept using it for a bathroom, no matter how often I cleaned it.
You might try an enzymatic clean especially designed for dog and cat urine. It eats up the odor with good bacteria and is effective on even a dog-and-cat smelling level. Another suggestion is to try Scrubbing Bubbles bathroom cleaner. That stuff is awesome! I've used it to pull stains out of carpet that no other cleaner could touch, and as an oven cleaner it was fantastic (without all that nasty oven cleaner smell).
You also might try renting a carpet cleaner with an upholstery attachment and purchase a stain-removal formulated carpet cleaner. That usually works pretty well.

Hmm...if the stain is pretty well saturated into the mattress, you may not have a choice but to trash the mattress and get a new one. You can try a chemical stain remover, but it only works on surface stains and not on stains that have soaked in pretty deep.

You should buy a mattress cover, either a plastic one, or one that for dust mites. This will keep the yuckiness away from you until you decide to get a new mattress. It's good for allergies too. You can get them pretty inexpensively at Walmart.

if you have had any changes in your life i.e. an new person, moved recently,etc. your cat may be trying to let you know there is a problem. they can suffer from depression,anxiety, and jealousy! if you have had any changes going on then that might be the culprit. not much will help you with the mattress but you should address your kitty's issues or else it will just continue.

I didn't have any success with a mattress though I tried Nature's Miracle and other things. I just bought a new mattress and chalked it up to "cat expenses -other".

mix 1 table spoon laundry detergent and 1 teaspoon of bleach.mix with 1"3 cup of water scrub on matress and let sit 30 min then rinse with wet cloths till clean...will smell good and get stains out,works for me and my 3 cats and 1 dog,,,,,,,,

Use bicarbobate of soda on the area. it's great for shifting stains, is cheap and will also remove the smell.
