It's just called a humidifier, steamer, or water pot. You can find them in lots of catalogs, like L L Bean, Plow & Hearth, etc.

Search decorative radiator covers. They can be custom made out of different materials, some come in kits. Believe it or not, there are a lot of websites out there for radiator covers. You could even design your own.
