Could be a number of things depending on what you mean. I could be not enough transmission fluid, but then you wouldn't have a transmission anymore. The clutch plate might need to be replace or the spring for it. You gear could be ground down to a nothing. You do not need a new radiator. The current one can be flushed and insured that there is not particle left, if the mechanic is legit. But it might need to be for other reasons. What work has been done to it recently?

Nissan exterra with the 5 -sp transmission
The clutch can be adjusted on this model
What happens is the clutch pedal looses free travel in the pedal
the way you check this is;
using you index finger with the car off and out of gear push down on the clutch pedal,if you feel presure on the pedal and its very hard to push the pedal down using your index finger,then it needs to be adjusted.
If you push the pedal down using your index finger and the pedal goes down aways then it gets hard to push down,that would mean the adjustment is ok.
To adjust the free travel you can look in the yellow pages under
Clutch Repairs and call them.Most clutch shops offer free clutch adjustments,or if you want to do it your self
Clutch adjustment;move the seat (drivers side)all the way back
using a flash light follow the clutch pedal to the firewall,you will see a shaft coming out and connecting to the clutch pedal.On the shaft there will be a lock nut you will need to losen up that nut.
now you need to turn the shaft counter clock wise to add free travel,if you turn the shaft clock wise you will take away free travel and you don't want to do that.That will cause the clutch to
slip(like the problem your having)
turn the rod until you feel with you finger the pedal goes down aways before it gets hard to push down.
I have a 2002 exterra and I love mine.I have the super charger on it.No major problems with mine at all.Well I just had to replace the battery.

You need to know if the transmission is actually slipping. Dealer can run the transmission codes and reccomend the proper action. Up to replacement of the transmission and radatior. Yes radatior has to be replaced to keep metal particals out of the new transmission. Pulling the transmision pan your self if you see metal front or rear of the pan your trans is done.

If it is an automatic then mabey the fluid is low, filter clogged. Manual then the clutch disk may be worn out of low clutch fluid.

possibly the gears have been grinding and have been shaved down too much. go to your local goodyear or jiffy lube or nissan dealer and they will help you.
