Is there ever been a case of siamese twins being a boy and a girl or is it always a boy and a boy or a girl and a girl?

its always boy boy or girl girl because the only time that you get boy-girl twins is if it is two different eggs that get fertilised.
Siamese twins happen when one egg is fertilised and doesnt seperate properly into individual twins.

It can only every be same sex, as it is the failure of a single egg to separate properly that results in siamese twins. Boy-girl twins develop from two separate eggs.

Didn't you see the Grey's Anatomy episode? They have to be the same sex because they come from the same egg.

It's always same sex because they come from the same egg which doesn't divide properly.
