Numerous reasons;
-sunlight; fades natural and dyed hair (hence the people who go really blonde by being out in the sunlight)
- condition of the hair; If your hair was porous and damaged its not going to hold the colour as well or as long than if it were healthy. I recomment treatments once weekly.
- products used; You should look for shampoos and conditioners (I stress the importance of professional products) made for colour treated hair, that usually contain moisturizing agents and UV filters, to help protect from sun damage/styling damage
-amount of times you wash your hair; Because it is an artificial colour, every wash rinses it out a little bit more (this goes for semi, demi and permanent hair colours)
-heat styling; the amount of heat styling you do has the same effect as sunlight in that it can make your colour dull.
Most colours last about 2 months. Regrowth should be touched up every 6-8 weeks. But no matter what, colours will always fade.
Colouring your hair is a pretty big commitment to keep it updated and fresh looking. You have to be willing to touch up your regrowth, invest in the proper products, have the time and money to spend on keeping up with your look.
If you're having trouble, try talking to your stylist about what you can do to get a colour that doesn't fade as noticably.

Either you have been utilising inexpensive/transitority dye or you've got an overly powerful shampoo. There are shampoo's for dyed hair you'll be able to purchase that don't seem to be so competitive to the dye. If you desire a quickfix then rub a few child oil into your hair earlier than you wash and it's going to make the shampoo much less of an obstacle.

Some we have to go back to the salon for touch-ups. It fades a little with each color. Try only washing your hair every other day. Actually your hair will be more healthy be the natural oils in it. I like John Frieda's new color glaze. It's a in the shower 3 minute color treatment. It really helps keep your color longer.

Mostly all hair colors are meant to last 4-6 weeks. Even the permanent color. Permanent only means it is not going to wash in in 2-6 shampoos. ITs just a natural thing for color to fade/ wash out. Just make sure you are using shampoo and conditioner that is for color treated hair

It all depends on how you take care of it and what type of color you use.
semi/demi-permenant/temp color- will always fade out almost completly
Permenant color will fade slightly over time.
The best wayto make it last is use a shampoo made for color treated hair.
Also any type of heat (curling irons, flat irons etc) and sun can fade hair so it is good to use a product that protects from UVA and UVB
