Its just understood that you shouldn't. Its a social norm, there are no formal sanctions against it.

No it is not against with law going into the opposite sex's bathroom as long as it is vacant. But if you go into the opposite sex's bathroom and you have an adultery in force you will be caught by police and be punished as criminal because you are against with law.It is only an ettiquete that someone should go into the right bathroom for man or for woman. It is a habit that a man should go into a bathroom for man only and not the opposite
Sometimes I entered to the wrong batrhroom because I did not aware to see its sign, but after I knew it I went back and entered the right one.

It depends on the jurisdiction - in my city (New York) it's a violation for a man to go in the ladies room or a woman to go in the mens room - the police can write you a summons and you'll have to pay a fine.
But, in practice, it really depends on WHY you go in the opposite sex's restroom.
If you're at the movies, and there are 60 women on line for the ladies room, and nobody on line for the mens room, and you need to urinate very badly, nobody is going to say anything about you going in the mens room - it happens all the time under those circustances, and you have a legitimate reason.
But, if you're going in there for an illegitimate reason (like you are going in the mens room to have sex with some guy, or to use drugs in the stall) you might get in trouble.

I am sure there is a law against it. It can be seen as a form of intrusion or assault I would think. It could also be looked upon as a form of stalking or even attempted rape in some cases. It certainly is deviant behaviour in most cases I feel. However, If you are a recidivist and you are constantly being caught. I am sure the law would look upon it harshly, and so they should. If you make a mistake and you walk in there, you should immediately walk out. Then there would be no issue really. However, if you linger, and are caught, you deserve to be punished. I think it is all about what intent you had at the time of entering. Who but a sick person or a person with criminal intentions would want to go in there anyway?
There sure are some sad people in this world.

I sure hope not. I do it all the time. If the men's room is open and the womens is full, I do not hesitate to go in. Although it is usually dirtier and smellier. When ya gotta go ya gotta go.

Yes, it is considered sexual harrasement. The supervisor should be notified first. If something is not done than you have legal rights.
